About the role
The Patient Mobilization Coordinator ensures seamless patient flow from identification through treatment and follow‑up across outreach locations and mobile clinics. This role bridges community outreach, clinical teams, and logistics to improve patient experience and support organizational growth targets.
Key responsibilities
- Coordinate patient identification, sourcing, and mobilization activities across outreach sites and mobile clinics.
- Manage the full patient care journey, including pre‑admission, admission, treatment, discharge, and post‑care follow‑up.
- Track patient pipeline metrics from referral to care delivery and generate conversion reports.
- Implement community awareness initiatives and strengthen referral networks.
- Organize patient transport logistics under CURE programs and support home‑visit follow‑up documentation.
- Conduct social‑work related activities that support recruitment and follow‑up.
- Collaborate regularly with Business Development, Programs, Clinical, and Booking teams to optimize scheduling and care pathways.
Required profile
- Experience in coordinating healthcare or community outreach programs.
- Strong organizational skills and ability to manage multiple stakeholder relationships.
- Excellent communication skills for patient interaction and feedback collection.
