Support Worker – Disability Care

About the role

The Well Team is seeking compassionate Support Workers to provide hands‑on assistance to NDIS participants across Sydney, Melbourne and Darwin. You will work directly in participants' homes and communities, helping them achieve greater independence and quality of life.

Key responsibilities

• Assist with personal care tasks such as hygiene, dressing, meal preparation and mobility support.
• Support participants in managing household chores and developing independent living skills.
• Administer medication and follow health management plans as directed by health professionals.
• Implement behaviour support plans and positive behaviour strategies.
• Communicate regularly with your team leader and record progress notes after each shift.
