NDIS Support Coordinator

About the role

The Well Team is seeking an NDIS Support Coordinator to help participants navigate the NDIS system, maximize their funding, and achieve personal goals. You will act as a trusted advocate, guiding participants through planning, service selection, and ongoing support.

Key responsibilities

• Collaborate with participants to understand their NDIS plans, goals, and support needs.
• Develop and implement comprehensive support coordination plans aligned with funding and participant priorities.
• Monitor service delivery, adjust plans as needs evolve, and ensure compliance with NDIS requirements.
• Prepare participants for plan reviews by gathering evidence, documenting outcomes, and identifying future needs.
• Maintain audit‑ready records and contribute to the Operations team’s high‑standard service delivery.

Required profile

• Strong understanding of the NDIS ecosystem and ability to simplify it for participants.
• Excellent relationship‑building and advocacy skills.
• Commitment to participant‑centred outcomes and continuous improvement.
• Ability to work collaboratively within a service‑delivery team.

Required skills

• Knowledge of NDIS planning, funding structures, and service coordination.
• Experience with audit documentation and compliance processes.
