Patient Mobilization Coordinator

About the role

The Patient Mobilization Coordinator will lead the identification, outreach, and full care journey of patients across CURE’s mobile clinics and community programs in Kiambu, Kenya. This role ensures seamless coordination from referral through treatment completion, supporting both clinical outcomes and patient satisfaction.

Key responsibilities

• Coordinate patient identification, sourcing, and mobilization across outreach locations and mobile clinics.
• Manage the complete patient care pathway, including pre‑admission, admission, treatment, discharge, and post‑care follow‑up.
• Track patient pipeline from referral to treatment completion and generate conversion reports.
• Implement community awareness initiatives and strengthen partnerships with local referral networks.
• Organize patient transport logistics and maintain robust communication and feedback systems.
• Assist with home visits, documentation, and social‑work related activities supporting patient recruitment and follow‑up.
• Collaborate regularly with Business Development, Programs, Clinical, and Booking teams to optimize scheduling and care pathways.
• Produce reports on mobilization, referral tracking, and conversion to guide operational decisions.

Required profile

• Mid‑level experience in patient coordination, community outreach, or health‑service logistics.
• Strong ability to work cross‑functionally with clinical and administrative teams.
• Excellent communication skills and a commitment to patient‑centered care.
• Background in social work or related fields is advantageous.
