Care Coordinator – Remote

About the role

The Care Coordinator is the central point of contact for patients, families, healthcare providers, and administrative teams. Working remotely, you will organize, streamline, and monitor patient care plans to ensure high‑quality, timely healthcare delivery.

Key responsibilities

• Coordinate communication between patients, physicians, specialists, and multidisciplinary care teams.
• Assist in building, updating, and tracking individual patient care plans and schedule follow‑up appointments.
• Maintain accurate electronic health records, process referrals, verify insurance eligibility, and respond to patient inquiries via phone, email, or chat.
• Connect patients with community resources, home‑care services, medical equipment providers, and financial support programs.
• Conduct routine check‑ins to monitor treatment adherence, address barriers, and educate patients on their care plans.
• Identify and flag treatment delays, care gaps, or operational issues to clinical leadership.

Required profile

• 3+ years of experience in US healthcare coordination, medical administrative support, patient navigation, or case management.
• Exceptional verbal and written communication with strong empathetic listening.
• Outstanding time‑management, attention to detail, and ability to manage multiple care tracks simultaneously.

Required skills

• Proficiency with Electronic Health Record (EHR/EMR) systems.
• Experience using HIPAA‑compliant communication tools.
• Microsoft Office Suite.
• Google Workspace.
