Patient Mobilization Coordinator

About the role

The Patient Mobilization Coordinator will lead the identification, recruitment, and movement of patients through CURE International’s outreach locations and mobile clinics in Kenya. This role ensures a seamless patient journey from initial referral to post‑treatment follow‑up, supporting both clinical outcomes and community engagement.

Key responsibilities

• Coordinate patient identification, sourcing, and mobilization across outreach sites and mobile clinics.
• Manage the full patient care pathway, including pre‑admission, admission, treatment, discharge, and follow‑up.
• Track patient pipeline metrics, from referral to care delivery, and generate conversion reports.
• Implement community awareness initiatives and strengthen referral networks.
• Organise patient transport logistics under CURE programs.
• Maintain patient communication and feedback systems to enhance satisfaction.
• Conduct home visits, documentation, and social‑work activities related to patient recruitment and follow‑up.
• Collaborate regularly with Business Development, Programs, Clinical, and Booking teams to optimise scheduling and care pathways.

Required profile

• Experience coordinating patient care journeys and community outreach.
• Ability to manage logistics for patient transport and home visits.
• Strong reporting skills to produce mobilization and conversion data.
• Excellent communication to maintain patient feedback loops.
