Registered Nurse – Home-Based Care

About the role

The Registered Nurse – Home‑Based Care delivers safe, evidence‑based nursing services directly in clients’ homes. You will assess patients, administer medication, educate families and collaborate with multidisciplinary teams to enhance health outcomes and quality of life.

Key responsibilities

• Conduct comprehensive nursing assessments and develop individualized care plans.
• Administer medications, perform wound, catheter, PEG feeding, tracheostomy and colostomy care.
• Monitor vital signs, document clinical observations and report changes promptly.
• Provide health education on disease management, nutrition and preventive care to patients and families.
• Maintain accurate, confidential nursing records and incident reports.
• Collaborate with doctors, physiotherapists, occupational therapists and caregivers in multidisciplinary care planning.
• Uphold infection‑prevention measures, ethical standards and patient safety at all times.

Required profile

• Diploma or Bachelor’s degree in Nursing.
• Registered with the Nursing Council of Kenya.
• Minimum two years of clinical experience, preferably in community or home health settings.
• Strong communication skills and ability to work independently in patients’ homes.
