RN Clinical Nurse - Home Health

About the role

Join UNC Health’s Home Health team as a Clinical Nurse providing one‑on‑one, holistic care to patients in the comfort of their homes. You will deliver high‑quality, compassionate nursing services to acute and chronically ill adults across the Raleigh and Chapel Hill regions.

Key responsibilities

• Perform comprehensive patient assessments and develop individualized care plans during home visits.
• Administer wound care, infusion therapy, IV line and catheter management.
• Document OASIS assessments and maintain accurate clinical records.
• Coordinate with interdisciplinary team members and communicate changes in patient status.
• Travel to assigned patient homes using personal vehicle and adhere to scheduled visit times.
• Participate in on‑call rotations (one weekday/week and one weekend/month) as needed.

Required profile

• Active NC Registered Nurse license.
• At least 1 year of nursing experience with acute or chronic patient care; 2–3 years Med/Surg preferred.
• Ability to work independently, manage own schedule, and travel within the assigned region.
• Personal vehicle with current auto insurance.
• Willingness to be on‑call for medically necessary situations.
