Care Manager (Remote – New York/New Jersey)

About the role

We are seeking dedicated Care Managers to join our remote team serving individuals with developmental disabilities and complex needs in New York and New Jersey. This full‑time role focuses on delivering person‑centered health home services and supporting members to access the care and resources they need.

Key responsibilities

• Deliver the six core Health Home services, including comprehensive care management and assessments.
• Develop and maintain individualized Life Plans for members.
• Coordinate care, promote health, and manage transitions after hospital or rehabilitation discharge.
• Provide individual and family support and refer members to community and social support services.
• Document activities in Electronic Health Records while ensuring HIPAA compliance.
• Conduct face‑to‑face visits based on member needs (monthly, quarterly, or bi‑annually).
• Monitor benefits such as Medicaid, SSI, and SNAP.
• Assist members with school‑related and personal planning needs.
• Report incidents of abuse, neglect, or maltreatment.
• Collaborate with internal and external interdisciplinary teams.

Required profile

• Bachelor’s degree plus 2 years relevant experience, or RN license plus 2 years experience, or Master’s degree plus 1 year experience.
• Must reside in New York or New Jersey.

Required skills

• Proficiency in Microsoft Office Suite.
