Medical Service Coordination Assistant

About the role

The Medical Service Coordination Assistant supports the efficient flow of patient referrals between healthcare providers and referring organisations. You will ensure that each referral is matched with the right service, tracked through to completion, and documented in line with regulatory standards.

Key responsibilities

• Review incoming referrals, assess patient needs and match them with appropriate providers based on service type, location and capacity.
• Facilitate communication among referring organisations, providers and internal teams to confirm care arrangements and provide status updates.
• Track referral progress from intake to discharge, monitoring deadlines and any changes in care plans.
• Maintain confidential, accurate records of referrals, communications and outcomes using EMR and other healthcare software.
• Verify provider qualifications, capabilities and availability to keep the network compliant and responsive.
• Generate documentation, trackers and stakeholder communications using Microsoft Office tools.

Required profile

• Entry‑level qualification with up to two years of relevant experience.
• Strong attention to detail and ability to manage multiple patient cases simultaneously.
• Professional, empathetic communication style for both patients and providers.
• Flexibility to shift priorities in a dynamic healthcare environment.
