Occupational Therapist – Home-Based Care

About the role

The Occupational Therapist will deliver home‑based rehabilitation services, helping clients improve independence, safety and overall quality of life. Working directly in clients' homes, you will assess functional abilities, create personalised therapy plans and collaborate with families and multidisciplinary teams.

Key responsibilities

• Conduct comprehensive occupational therapy assessments covering physical, cognitive, sensory and functional domains.
• Develop and regularly update individualized treatment plans with measurable goals.
• Provide hands‑on therapy interventions for activities of daily living, recommending adaptive equipment and assistive devices.
• Perform home safety assessments and suggest modifications to reduce fall risk.
• Maintain accurate clinical records, assessment reports and progress notes.
• Collaborate with nurses, physiotherapists, speech therapists, doctors and caregivers in multidisciplinary case discussions.

Required profile

• Bachelor’s degree in Occupational Therapy.
• Registered and licensed to practice in Kenya with a current practising licence.
• Minimum of 2 years of clinical experience (home‑care experience is an advantage).
• Valid driving licence.
