Registered Nurse – Home & Community Care

About the role

The Turker Foundation is seeking a full‑time Registered Nurse to deliver direct patient care in home and community settings within Homa Bay County, Kenya. You will work on‑site, using digital tools to document care, schedule visits and coordinate with a multidisciplinary team.

Key responsibilities

• Provide comprehensive nursing assessment, diagnosis, planning, implementation and evaluation for patients at home.
• Administer medications, monitor treatment outcomes and vital signs.
• Educate patients and families on health promotion, disease prevention and self‑care.
• Collaborate with physicians, pharmacists and other health professionals to ensure integrated care.
• Use electronic health records and mobile applications for documentation, scheduling and communication.
• Follow clinical guidelines, infection‑prevention protocols and patient‑safety standards.
• Participate in community outreach programs and continuity‑of‑care initiatives.
• Contribute to quality‑improvement projects and data reporting.

Required profile

• Current registration and licensure as a Registered Nurse with the Kenyan nursing council.
• Solid clinical nursing skills, including patient assessment, care planning and medication administration.
• Knowledge of community health, home‑based care and chronic disease management.
• Ability to work independently in home and community environments and manage time effectively.
• Diploma or Bachelor’s degree in Nursing; additional training in community health is an advantage.
• Experience in pharmaceutical or integrated‑care settings is beneficial.
