Community Health Worker

About the role

IMP aCT Care is seeking a full‑time Community Health Worker to join its Kaiser Permanente partnership. You will work directly in the community, building trusted relationships with high‑risk patients and linking them to the care and resources they need.

Key responsibilities

• Build trust through consistent, culturally aligned communication.
• Identify and address barriers such as food insecurity, transportation, and housing.
• Connect patients to clinical teams and community resources, and follow up to ensure needs are met.
• Document all interactions, assessments, and outcomes in the Epic EMR system.
• Complete SDOH screenings and update care plans promptly.
• Participate in integrated care huddles and collaborate with clinical staff.
• Use performance data and feedback to refine approaches and improve outcomes.

Required profile

• High school diploma or equivalent.
• 1–3 years of community‑based outreach, health education, or related experience.
• Valid driver’s license, personal vehicle, and car insurance.
• Ability to pass background check and pre‑employment screening.
• Flexibility to cover multiple locations within the assigned region.

Required skills

• Experience with Epic EMR system.
• Familiarity with SDOH screening tools such as Hunger Vital Sign and USDA Household Food Security.
• Knowledge of HEDIS metrics and risk stratification concepts.
